Can or should there be a standard letter requesting relief?

Answer:
Items to be addressed in the request email are:
a) Circumstances why proposed individual is not available.
b) Circumstances why other equal to or better individuals are not available, including all subconsultants.
c) Statement that replacement inspector can adequately perform necessary inspection duties.
d) Resumes for replacement inspector and the original inspector are to be included
e) All requests should be submitted via email to PennDOT at the following email box: RA-pdconsult_waiver@pa.gov
