General Information - It asks for the business address. Is this the business address of the parent company (who most likely will not be asking for work in Pennsylvania) or the main office in Pennsylvania? The business has multiple offices in Pennsylvania.

• Answer:
The address on the first page is for the corporate office. The following pages should contain the addresses that are doing business in Pennsylvania.
